Social Prescribing and Self-Care

The local voluntary and community sector provides a wide range of services to support the health and wellbeing of residents in Kensington & Chelsea, Queens Park and Paddington. It also works closely alongside colleagues from the Council and local NHS services to help inform the local health agenda. Currently, Social Prescribing is on the National agenda but what is it? 

What is Social Prescribing? What is Self-Care? 

KCSC currently manage three Social Prescribing schemes and a Directory: Social prescribing works alongside and supports traditional medical interventions by linking people up to voluntary and community groups and other non-clinical services. Social prescribing is a way to help people make positive changes in their lives by giving them the knowledge, skills and the confidence to do so. Self-Care is the action that people take to maintain and improve their own health and wellbeing.

 

My Care My Way
My Care My Way (MCMW) is a programme for anyone aged 65 and over. It is for people registered with a GP in Kensington & Chelsea, Queens Park or Paddington.
Community Living Well
Community Living Well is a programme for people aged 16 and over who need support with their mental health. It is for people registered with a GP in Kensington & Chelsea, Queens Park or Paddington.
Healthier Futures (North Kensington)
Healthier futures aims to support the communities in North Kensington affected by the Grenfell Tower fire.
Self-Care Directory
The Social Prescribing (Self-Care) directory has a list of services in the local community that residents can access to help take care of their health and wellbeing.
