What is Social Prescribing? What is Self-Care? 

KCSC supports local health and wellbeing through social prescribing and self-care initiatives, connecting residents to a range of non-clinical support within the community.

Social prescribing works alongside traditional medical interventions by linking people to voluntary and community services that can improve their wellbeing. It helps people make positive changes in their lives by developing the knowledge, confidence, and support networks they need.

Self-care means intentionally taking time to care for your physical, mental, and emotional health—through social connection, creative activity, exercise, mindfulness, and more.

KCSC currently manages one active social prescribing scheme and maintains a community self-care directory:

  • My Care My Way

Until March 2025, KCSC also delivered the Healthier Futures programme in North Kensington and previously supported the self-care element of Community Living Well.

My Care My Way
My Care My Way (MCMW) is a programme for anyone aged 65 and over. It is for people registered with a GP in Kensington & Chelsea, Queens Park or Paddington.
Healthier Futures - Now Conluded
The Healthier Futures programme ended on 31 March 2025 after five years of supporting the wellbeing of North Kensington residents affected by the Grenfell Tower fire.
Self-Care Directory
The Social Prescribing (Self-Care) directory lists local community services that residents can access to support their health and well-being.

Case Studies
In 2023-2024, over 3,000 people were referred to self-care services. Since the program began, there have been more than 10,000 referrals and over 30,000 sessions delivered. Below are recent case studies showing how these services have positively impacted local people.
