Health Help Now App

The ‘Health Help Now App’ is a National health resource that is being rolled out across the country by each Clinical Commissioning Group (CCG). With your permission, we would like to include your organisation in it. The App will help people in the community search for and access health and wellbeing services provided by the NHS, privately or locally by the Voluntary and Community Sector. It is a good opportunity to promote your services and our sector as a whole!

KCSC's Self care event success

KCSC runs a Self Care programme to offer Social Prescribing options to patients over 65 years old in the area, working with local organisations.

Voluntary and community organisations provide a range of services that contribute to greater health and wellbeing. These include services that are condition specific, like dementia support, relate to physical health such as exercise programmes and those that deal with aspects of mental health, such as befriending services or information and advice.

KCSC has launched its Self-Care directory online.

Social Return on Investment (SROI) report

Health and social care services in North and West London are building positive new models of cross-sector working to make better use of joined-up resources. The Self-Care social prescribing model has led to reduced avoidable need for hospitalisations, reduced need for GP practice hours, and reduced levels of physical pain and depression for patients.
