Forever Care is a new NHS project focusing on health inequality, particularly concerned with long term users of health services whose needs are often unaddressed by agencies and support systems which are isolated from each other. KCSC have been working with The Violet Melchett Health and Wellbeing Hub to develop an expression of interest format for the project.
Forever Care is aimed at facilitating a multi disciplinary team (MDT) for those living and accessing support in the south of the borough specifically. It will bring together key partners to develop an integrated local community of practice, with personalised health and wellbeing plans, ongoing support and care to address the complex needs of the most vulnerable local people.
To deliver the 18 month project, a local provider will co-ordinate a team made up of professionals from the voluntary sector and health and social care.
Whilst we recognise our role in providing support on the EOI, we are keen to explore a joint bid with interested organisations or support a local VCS organisation to submit an expression of interest.
