A new service leaflet is being distributed to over 33,000 households and businesses in North Kensington reminding people who live or work in North Kensington and have been affected by the Grenfell Tower fire that the NHS is here to support the community and that there are a wide variety of extra services they can access for free to support their physical health and wellbeing.
These are all detailed in the new service leaflet and include:
- Dedicated service for survivors and bereaved and residents of Grenfell Walk
- North Kensington GP care
- Enhanced health checks
- Self-care support
- Wellbeing services provided by Central North West London NHS Foundation Trust through the Grenfell Health and Wellbeing Service.
The leaflet also highlights the extra support available for people with a chronic disease or long-term health condition where we will work closely with the patient, adult social care, social prescribing link workers, as well as the hospitals, GPs and other healthcare teams, to build a personal care plan.
Patients can be seen in the Integrated Care Centre at St Charles Centre for Health & Wellbeing, at a GP surgery, or in their own home if they are unable to attend a clinic.
The service is provided through the Grenfell Community Case Management Service (GCCMS), provided by Central London Community Healthcare NHS Trust (CLCH). Patients can be referred by their GP and can also self-refer.
The leaflet also references there are a range of services and support available through the NHS, and voluntary and community organisations for children and young people up to age 18.
They've also produced a poster advertising Community Enhanced Health Checks to everyone and an additional one saying the ‘NHS is here for you’.
The latter is provided by Central North West London NHS Foundation Trust through the Grenfell Health and Wellbeing Service.
