Healthwatch England has launched a survey asking people to share feedback about their hospital discharge experiences, or the experiences of someone they care for.
This survey is for people who have had a difficult experience of leaving hospital in the last 12 months, including where discharge was delayed, happened before they felt ready, or where support, equipment, information or follow-up arrangements weren’t in place.
