North West London Integrated Care Board is funding KCSC, SMART and Violet Melchett Health & Wellbeing Centre to develop a pilot of a project for patients with unmet needs known as Forever care
Forever Care aims to design a service that addresses needs of people in Chelsea / Brompton area who:
- Suffer health inequalities - characteristics which make them more likely to suffer health problems or difficulties in accessing healthcare - for example English proficiency or domestic circumstances
- Have ongoing health and wider social issues that are likely to continue
- Have not had their needs met by short term interventions or services have otherwise not met their needs
The project will be a small pilot with around 5 patients until spring 2026 and around 20 patients throughout in 2026. If the project shows an impact it is hoped to expand it and fund a full service and also share learning and implement changes to other services.
SMART will be co-ordinating a Multi-Disciplinary Team to identify suitable clients and design suitable services for them. This team will include health professionals and local community organisations. The Terms of Reference for the team are being updated and will be shared here.
We are looking for participation form local community organisations to:
- Help identify suitable patients for the pilot
- Help identify gaps in local services
- Participate occasionally in MDT if appropriate
- Potentially support patients themselves to attend MDT for their case
- Offer suitable support as part of the forever care project
- Contribute to the evaluation of the project
KCSC will be supporting local community groups to participate in the project. This will include
- Supporting community groups to participate in MDT
- Payments for time taken
- Training / induction
- Other support identified with community
- Support to implement privacy and safeguarding polices if needed
- Small grants to deliver services
- Support in developing governance procedures if needed
