Collaborating to deliver health and wellbeing Self Care services in the future

Come along and help shape the future delivery of self care services for older people. We are keen to have a wide representation from across the voluntary sector.

 

Background: Kensington and Chelsea Social Council is the accountable body responsible for delivering the Self Care pilot on behalf of NHS West London Clinical Commissioning Group. The pilot is now in its second year and there are lessons learnt that will inform the way forward. The programme is aimed at helping patients aged 65+ with long-term conditions by supporting them to engage with self-care services, including those provided by the voluntary sector. Self-care helps to promote individual empowerment, increase confidence and help people to be better informed about the steps they can take to manage their personal health and wellbeing.  Self Care is part of a wider integrated care initiative known as ‘My Care, My Way’ (MCMW). MCMW is a pioneering multi-organisation collaboration bringing together GP surgeries, NHS hospitals, social care services and local voluntary organisations to deliver support and care for local patients.

WHO IS IT FOR?

This workshop is targeted at organisations delivering services to older residents in Kensington and Chelsea, Queens Park or Paddington, not currently involved in the Self Care pilot. Our objectives are to:

  • Find out which organisations might be interested in contributing towards the Self Care contract;
  • Understand what support organisations need in order to play a part in a future consortium.

AGENDA

  • Overview of current non-clinical models of care
  • WLCCG Self Care model and contract-readiness requirements
  • Mapping of potential new self care providers/services
  • Understanding support needs of organisations
  • Expression of interest and next steps.

Lunch will be provided.
