If you work with individuals that have long term conditions like diabetes, dementia or chronic heart disease come along to this event to help design the best ways to deliver self-management support.
This engagement event is organised by the Hammersmith and Fulham, West and Central London Clinical Commissioning Groups. They are keen for the local voluntary sector to come along to input their views together with any service users that are able to attend and share their experiences.
Places are limited so do book early!
The event is open to any voluntary or community organisation operating in the boroughs of Kensington and Chelsea, Hammersmith & Fulham or Westminster as well as local patients.
