The NHS Long Term plan has been drawn up by frontline health and care staff, patient groups and other experts. They have benefited from hearing a wide range of views, whether through the 200 events that have taken place, and or the 2,500 submissions they received from individuals and groups representing the opinions and interests of 3.5 million people.

Personalised Care aims to benefit up to 2.5 million people by 2024, giving them the same choice and control over their mental and physical health that they have come to expect in every other aspect of their life. The document sets out how the NHS Long Term Plan commitments for personalised care will be delivered.
