NHS Funded Providers Forum

Purpose:

  • Funded providers to network and discuss how their work is going, develop partnerships and improve relationships with commissioners and NHS providers.
  • Platform for NHS Kensington & Chelsea to communicate to all the organisations around performance management issues, forthcoming commissioning opportunities, etc.

ALL ORGANISATIONS FUNDED BY NHS KENSINGTON & CHELSEA ARE INVITED TO JOIN THIS FORUM.

KCSC support for funded organisations from now until March 2011 (end of the programme)

This proposed package of support to commsisioned organsiations was agreed at the forum meeting on 16 June. KCSC staff will now be arranging to meet with all organsiations individually over the coming weeks, and takijng forward the devleopment work that is outlined in the proposal. We will also be working with colleagues at NHS Kensington & Chelsea, who we hope will continue to offer training in public health and health promotion skills.


 Community Health Consortium

  • KCSC is building a voluntary sector consortium to work in partnership with the local NHS to develop two new primary care facilities - one at St Charles Hospital in North Kensington and one at Hogarth Road in Earls Court.

Dalgarno Neighbourhood Trust - 'SHOUT' project

The SHOUT project offers local families a tailored plan with frequent one to one support, which encourages healthy eating, regular exercise and overall emotional wellbeing.

Family Friends

Befriending and mentoring services to families with children (0-16) through a network of trained volunteers.

Healthier Life 4 You (formerly Umoja Pamoja)

Supporting parents to better educate their children on primary health matters.

Independent Care Services

Providing practical care for the elderly by promoting healthier lifestyles, exercise and reducing isolation through education and information.

K&C Forum for Older Residents

An independent forum for senior residents offering opportunities to influence local policy and service provision.

A new service employing three community development workers in community centres across the borough, aiming to reduce stigma of mental illness, and reduce the high level of mental health problems among black and minority ethnic communities.

Living Well - Life Clubs

A workshop programme for young people focused on improving sexual health, confidence, motivation and their ability to make healthy choices in every aspect of their lives, whether it's to do with sex and relationships or work and money.

An interactive text service which provides young people with unbiased, confidential, personalised information, support and advice to help them make fully informed decisions regarding sex, sexuality, contraception, relationships and sexual health.

Midaye, Somali Development Network

Improving access to GP practices and health screenings among the Somali community in North Kensington througha programme of community workshops and one-to-one support.
